Applying is easy using our Online Application. A non-refundable fee of $75 must be submitted with each application.
Create an Online Application:
Birth Certificate UPLOAD REQUIRED ALL GRADES
Current Health / Immunization Form UPLOAD REQUIRED ALL GRADES
Educational/Diagnostic Testing Results (ie Speech or OT evaluations) and/or IEP (UPLOAD REQUIRED ALL GRADES if applicable)
Current Academic Transcript/Report Card/Progress Report UPLOAD REQUIRED Gr K-8
Teacher Recommendation REQUIRED Gr K- 8, submit current teacher name and email
Discipline Release Form REQUIRED Gr 1-8, submit school administrator name and email
Standardized Testing Scores UPLOAD REQUIRED Gr 3-8
Baptismal Certificate (if baptized)
